‘The combination of radiofrequency and ultrasound energy is innovative as it enables us to perform fat reduction and skin tightening in the same treatment,’ explains Sydney cosmetic physician Dr Phil Caraiscos. ‘If you were to just reduce the fat alone, you would be left with saggy wrinkly skin around the area of reduction, which sometimes occurs after liposuction.’

Exilis targets the radiofrequency and ultrasound energy at specific depths in the subcutaneous fat. This causes deep thermal heating to accelerate metabolic activity and initiate lipolysis – the death of fat cells.

‘In terms of fat reduction, we have seen very good results on the stomach, inner thighs, outer thighs, love handles and double chins,’ Dr Caraiscos explains. ‘For fat removal we target each layer of fat separately. The first pass targets deep fat with the machine set to full power and the cooler set to full. We then reduce the settings to target the middle fat layers, and then reduce them again for a third pass to target the superficial fat.’

Skin tightening

As the fat cell volume declines, the surrounding collagen is also affected. The heat triggers the contraction of collagen fibres in the skin and promotes the production of new collagen at the site – for a skin tightening and rejuvenating effect.

To treat skin laxity on the face, Exilis uses Collagen Refill Technology. The combination of radiofrequency and ultrasound energies quickly and efficiently dissociates the collagen structure under the skin. As a result, the natural healing process evokes the fibroblasts to produce new collagen fibres and the skin structure is eventually refilled with collagen to produce a tauter and more defined facial structure.

‘As skin ages, the bundles of collagen and elastin fibres unwind,’ Dr Caraiscos explains. ‘The skin tightening effect from Exilis occurs due to the focused radiofrequency energy elevating the tissue temperature and having a twofold effect: we see an immediate tightening from the winding up of these bundles, then, in the medium term, we see further tightening occur due to new collagen being produced in the skin. We have found the results with skin tightening to be remarkable.’

Exilis is suitable for use on all skin types and typically four treatments are recommended, usually one to two weeks apart.

Results

The full results of fat reduction and skin tightening are usually seen three to six months after treatment, once collagen synthesis has occurred and the fat cells have been metabolised by the body.

‘Some results are seen almost immediately, and improvements are seen for a further six months,’ Dr Caraiscos says. ‘For fat removal, results can be permanent if there is no more weight gain. For skin tightening, a top-up treatment is usually needed every one to five years.’

The treatment itself requires no anaesthesia and should not involve any downtime. It can be used in conjunction with other body contouring or skin resurfacing methods to achieve optimum results.
